Registered Nurse (Patient Aligned Care Team RN Care Manager)
This Registered Nurse (RN) is responsible for the coordination of care and the provision of care in the primary care setting.
This nurse executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines and influence organizational mission and health care.
This nurse provides communication between Veterans, Veteran Affairs Medical Center (VAMC) staff, providers, and community resources.
This nurse executes leadership that is characterized by substantial and continuous responsibility, facilitating consults, participating in and facilitating telehealth.
The RN understands and promotes both team and disease based care management to patients assigned to Primary Care at the Saginaw VAMC and/or Community Based Outpatient Clinic's (CBOC).
The RN Care Manager utilized clinical video telehealth equipment to facilitate primary care remote visits.
He or she also facilitates patient care and contribution to systems redesign patient flow and customer service goals.
The nurse must demonstrate leadership in delivering and improving holistic care to the adult and geriatric population through collaborative strategies and in compliance with nurse practice acts, in addition to medical center and department standards.
This nurse works under the guidance and direction of the CBOC Nurse Manager and may be requested to provide coverage at another CBOC or at the Saginaw Medical center.
The duties of the RN include, but are not limited to:
o Provides leadership in the application of the nursing process to patient care, organizational, process, and/or systems improving outcomes at a program or service level.
o Supports and enhances client self-determination.
Provides leadership in identifying and addressing ethical issues that impact client and staff, including initiating and participating in ethics consultations.
o Manages program resources (financial, human, material, and informational).
o Implements an educational plan to meet changing programs or service needs for self and others.
Maintains knowledge of current techniques, trends, and professional issues.
o Uses professional standards of care and practice to evaluate programs and/or service activities.
